By entering the “Submit Your Forms” Contest (the “Contest”), you agree to abide by these Official Rules and the decisions of the “Sponsor” which shall be final and binding on all matters pertaining to this Contest. Contest is subject to all applicable federal, state, and local laws.

Contest Description

The object of this Contest is to provide Delphia Consulting with examples of performance appraisal and general employee status change forms to be used by Delphia Consulting in its usability testing of HR Actions, and HR Actions Talent Management. For the purpose of this Contest, the forms you submit in this contest along with your email address (see below) will be called an “entry.”

Eligibility

You are eligible to enter this Contest if you:

1.       Are a legal resident of the 50 United States or the District of Columbia; and

2.       Are at least thirteen (18) years old at the time of; and

3.       Have an active email address; and

4.       Are not employed by Sponsor or any of its parents, affiliates, subsidiaries, participating vendors or advertising or promotion agencies, or anyone working directly on this program, product, and promotion (collectively the “Released Parties”); and

5.       Are NOT an immediate family member (parent, child, spouse, sibling, and their respective spouses, regardless of where they reside or a person living in the same household, whether or not related), of any of the employees of the Released Parties.

Start and End Dates

The Contest begins at 12:00:01 A.M. on May 1, 2012 and ends at 11:59:59 P.M. on October 31, 2012 (“Entry Period”). The Entry Period will be divided into: Daily Entry Periods. All entries must be received by the specified deadline for the applicable Entry Period to be eligible; any entries received after the applicable deadline will be void and will not be considered for the prize drawing.

Entry Requirements

In order to be eligible for the drawing, an entry must meet the following requirements: Your entry must contain a valid email address and submit an example of a blank performance appraisal form and a blank employee status change form used within your workplace. (Please do not submit form that have been completed).

How to submit your entry

To enter, provide your email address, your example form, and click the “Submit” button. Limit one entry per e-mail address per contest period. Additional entries received from any e-mail address after the first entry will be void. Sponsor reserves the right to determine the validity of any entry, the correctness of entries, and their decisions are final and binding in all matters pertaining to this Contest. Lost, late, incomplete, inaccurate, stolen, misdirected, undelivered, and/or garbled entries as well as entries generated by any script, macro, or other automated means will be disqualified.

Drawing

Contest winners will be determined based on a drawing from the qualified entries deemed to contain the valid example forms. The drawing will be done by November 15, 2011.

Winner Notification

If you are determined to be a potential winner, You will notified via an email sent to the email address you submitted as part of your entry; and you may be required to sign and return an Affidavit of Eligibility/Liability/Publicity release, unless prohibited by law, within ten (10) days of date of prize notification. If you are a potential winner and you (i) do not reply to such prize notification or any prize notification is undeliverable, (ii) do not return the Affidavit of Eligibility/Liability/Publicity release completed and signed within ten (10) days of date of prize notification, or if you (iii) are not otherwise in compliance with these Official Rules, you will be disqualified and, we may, at our discretion, notify a runner-up. If you are a winner and accept the prize, you agree that we and our designees shall have the right to use your name, city and state of residence, voice, likeness, and/or photograph for advertising and promotion purposes, in any and all media now or hereafter known worldwide in perpetuity, without additional compensation, notification, or permission, unless prohibited by law.

Prize

The prize is a $500 Visa Gift Card or equivalent. If you are a winner and you do not wish to or cannot accept the prize you may redirect your prize to a charitable 501(c)(3) of your choice. We reserve the right to substitute a prize with one of comparable or greater value, at our sole discretion; and are responsible for all federal, state, and local taxes (including income and withholding taxes) as well as any other costs and expenses associated with accepting and/or using the prize that are not specified herein as being part of the prize award; and you understand you are accepting your prize “as is” with no warranty or guarantee, either express or implied by us; and You understand that all prize details shall be determined solely by us.

Limits of Liability

All entrants agree to be bound by these rules and agree to hold Sponsor, its affiliates, sponsors, independent contractors, distributors, directors, administrators, respective officers and employees free from any liability for injuries or damages of any kind while involved in a contest activity or through the use of this program.

General Conditions

This Contest is governed by Ohio law. You agree that the jurisdiction and venue for the handling of any disputes or actions arising out of this Contest shall be in the courts of the State of Ohio without giving effect to any choice of law or conflict law rules (whether of the State of Ohio or any other jurisdiction) which would cause the application of laws of any jurisdiction other than the State of Ohio. If, for any reason, the Contest is not capable of running as planned for any reason, including, without limitation, the reasons set forth above, we reserve the right at our sole discretion to cancel, terminate, modify or suspend the Contest (or portion thereof). If Contest (or portion thereof) is cancelled or terminated, we may, at our sole discretion, determine the winners of this Contest for the Entry Period(s) at issue using all non-suspect, eligible votes and/or entries received up to time of cancellation or termination for the applicable Entry Period(s). We may disqualify you from participating in the Contest or winning a prize (and void your participation in the other promotions we may offer) if, in our sole discretion, we determine you are attempting to undermine the legitimate operation of the Contest by cheating, deception, or other unfair playing practices, or intending to annoy, abuse, threaten, or harass us, any other entrant, or our representatives or if you are otherwise not in compliance with the terms of these Official Rules. CAUTION: ANY ATTEMPT BY YOU OR ANY OTHER INDIVIDUAL TO DELIBERATELY DAMAGE ANY WEB SITE OR UNDERMINE THE LEGITIMATE OPERATION OF THE CONTEST IS A VIOLATION OF CRIMINAL AND CIVIL LAWS AND SHOULD SUCH AN ATTEMPT BE MADE, WE RESERVE THE RIGHT TO SEEK DAMAGES AND OTHER REMEDIES FROM YOU (INCLUDING ATTORNEYS’ FEES) TO THE FULLEST EXTENT PERMISSABLE BY LAW.
